U.S. Passport Acceptance Office

The Office of Continuing Education & Workforce Development (OCEWD), at Leeward Community College, is a designated Passport Acceptance Facility. You can easily obtain your passport through our office.

Only form DS-11 (for first-time applicants, minors under the age of 16, and all other who do not qualify to use the renewal form DS-82) is accepted at this facility. All mail-in applications should be mailed to the appropriate address on the application form.
Leeward Community College DOES NOT accept form DS-82. Only form DS-11 is processed here. Mahalo for your understanding.

Payment Information

Two SEPARATE payments must be made:
1) Passport Fee (adult): $110 - Personal check or cashier's check only - make check payable to "Department of State"
2) Passport Fee (minor): $80 - Under age 16
3) Execution fee: $35 - Cash, credit card, money order or cashier’s check only (NO PERSONAL CHECKS)
